New York Work Permits
Similar to California, New York has two kinds of work permits for young performers but the timing is a little different:
1. 12 Month Permit (application here)
2. 15 Day Permit (first time applicants only, application here). This permit is available online, and you will receive it immediately.
New York Department of Labor issues the permits:
Division of Labor Standards
Permit and Certificate Unit, Room 185B
State Office Campus, Bldg 12
Albany, NY 12240
NYS Child Performer Unit (518) 457-1942
Dept. of Labor (518) 457-9000
Email for quick turnarounds and other emergency situations: CPinfo@labor.ny.gov
To obtain a permit you must complete and application and attach several forms: an annual physical (usually the same one that New York kids must have for school), school clearance, a trust account form (in NY, it the account is not blocked but is a trust account), and an emergency contact form. The Department of Labor has a new website with all the info you need here.
Regulations for Child Performers in New York are called Part 186 and they can be downloaded in total here..
