New York Work Permits
Similar to California, New York has two kinds of work permits for young performers but the timing is a little different:
1. 12 Month Permit (application here)
2. 15 Day Permit (first time applicants only, application here). This permit is available online, and you will receive it immediately.

To obtain a permit you must complete and application and attach several forms: an annual physical (usually the same one that New York kids must have for school), school clearance, a trust account form (in NY, it the account is not blocked but is a trust account), and an emergency contact form. A Note About Models
As of 2013, print and runway models are expressly included in the child performer regulations in New York. Those who live or work in New York state must comply. FAQs specifically for models are here.
